Malinoski & Associates, D.D.S., P.C., is always accepting new patients.  Our policy requires a new patient to fill out a patient registration form and a health history.  These forms must be returned to our office before we can schedule an appointment.  We have attached a printable version of the forms for your convenience.  Fill these out and return them in person or via mail.  If they are mailed to us, once we receive them, someone from the office will phone you to set up an appointment.
