We
see all patients on an appointment basis, and ask that you call
in advance so that we may reserve the time for you. The office
phone number is (269) 279-7876. We try to see all patients on
time and request that you extend the same courtesy to us. If
you can not keep an appointment, call 279-7876 to notify us
immediately. Please give us 24 hours notice so that this time
may be given to another patient. We reserve the right to charge
you for missed appointments if they become a recurring problem. |

office is open on the following schedule:
We are closed every day from noon to 1:00 pm for lunch. We are
also closed on major holidays and at times when we are away
attending continuing education programs. During the week before
or after a major holiday, please be aware that our office hours
may change. For updated information please call our office. |

Care |

We
realize that emergency situations do arise and we will do everything
in our power to respond to your problem as quickly as possible.

If an emergency arises while the office is closed, call the
doctor at home. |
| |

If an emergency arises while the office is open, please call
the office as early in the day as possible. We reserve special
times for those patients with emergency needs and you will be
seen as quickly as possible. |

encourage you to call with questions you may have concerning
dental problems; however, it would be unfair to other patients
if the doctor were to disrupt treatment to answer most questions.
If your call requires that the doctor speak with you, please
leave a message and the doctor will return your call at the
earliest opportunity - usually at the end of the business day. |

We
make every effort to keep down the cost of your dental care.
If your treatment program requires several visits, you will
be given an estimate. Payment will be due for all services on
the day services are performed. We accept cash, checks, American
Express, Discover, Mastercard, and Visa. |

We
also realize that many families are in a state of change. Divorced,
separated, single-parent, and blended families are now common.
In many of these families the question of the children's account
is uncertain. The policy in our office is that the parent seeking
treatment for the child is responsible for all fees incurred. |

| Insurance |

Dental
insurance is intended to cover some, but not all, of the cost
of your dental care. Most plans include coinsurance, a deductible
and other expenses which must be paid for by the patient. |

If
you have dental insurance, please bring your plan information
and signed, completed insurance forms with you to your first
visit. We will work with you to assure that you receive the
maximum benefits to which you are entitled. |

We
file your insurance claims as an additional service for you.
We want to make sure you understand that payment for services
is your responsibility. Any questions you have regarding your
insurance benefits should be directed to your employer's insurance
representative. |
